THROUGH A GROUP POLICY ISSUED TO TAR BY DELTA DENTAL INSURANCE COMPANY (Delta is one of the largest dental carriers in the United States
and provides over 23 million people with Dental Benefits).
 
The two plans are as follows:
REALTORS® PPO PLAN
*  Enrollee receives lower out of the pocket cost by using a Delta Dental Provider where
    procedures cost have been negotiated.
 
REALTORS® INDEMNITY PLAN
*  Enrollee may use the dentist of their choice and benefit payment will be made on “usual and
    customary charges” in the geographical treatment area.
 
 
  1. Maximum benefits up to $1,200 per insured per year.
 
  1. Benefits increase after 1st year in selected plan.
 
  1. Enrollment available regardless of age.
 
  1. Enrollment available regardless of any pre-existing condition.
